Purpose<\/h2>\n

Use this procedure to create a form for a site inspection, pre-use equipment check, toolbox talk, sign-off sheet, evidence record or another CHKM8 workflow.<\/p>\n

Steps<\/h2>\n

  1. In WordPress Admin, open Forms \u2192 New Form<\/strong>.<\/li>\n
  2. Choose a template if a suitable one is available, or choose a blank form.<\/li>\n
  3. Enter a clear name, for example Tracked Stump Grinder \u2013 Daily Check<\/em>. Use a name that users can recognise in menus and entry reports.<\/li>\n
  4. Click Create Form<\/strong>.<\/li>\n
  5. In the Form Editor, add fields from the field library. Click a field to edit its label, description, required status and other settings.<\/li>\n
  6. Arrange fields in the order the user should complete them. Group related fields together and keep the mobile workflow short.<\/li>\n
  7. Click Save Form<\/strong>.<\/li>\n
  8. Open the form settings and configure confirmations and notifications before making the form available to users.<\/li>\n<\/ol>\n

    Recommended structure for a CHKM8 inspection form<\/h2>\n

    1. Form title and short instructions.<\/li>\n
    2. Operative name and asset\/equipment ID.<\/li>\n
    3. Date and site\/job reference.<\/li>\n
    4. Inspection questions with clear answer choices.<\/li>\n
    5. Conditional fault description and evidence upload.<\/li>\n
    6. Corrective action \/ supervisor review, if required.<\/li>\n
    7. Signature or acknowledgement, if the workflow requires it.<\/li>\n<\/ol>\n

      Test before publishing<\/h2>\n

      Preview the form on desktop and a phone-sized screen. Submit a test entry using normal answers, then submit another using any fault or exception path. Confirm the entry is saved and the expected notifications arrive.<\/p>\n
