Purpose
Use this procedure to create a form for a site inspection, pre-use equipment check, toolbox talk, sign-off sheet, evidence record or another CHKM8 workflow.
Steps
- In WordPress Admin, open Forms → New Form.
- Choose a template if a suitable one is available, or choose a blank form.
- Enter a clear name, for example Tracked Stump Grinder – Daily Check. Use a name that users can recognise in menus and entry reports.
- Click Create Form.
- In the Form Editor, add fields from the field library. Click a field to edit its label, description, required status and other settings.
- Arrange fields in the order the user should complete them. Group related fields together and keep the mobile workflow short.
- Click Save Form.
- Open the form settings and configure confirmations and notifications before making the form available to users.
Recommended structure for a CHKM8 inspection form
- Form title and short instructions.
- Operative name and asset/equipment ID.
- Date and site/job reference.
- Inspection questions with clear answer choices.
- Conditional fault description and evidence upload.
- Corrective action / supervisor review, if required.
- Signature or acknowledgement, if the workflow requires it.
Test before publishing
Preview the form on desktop and a phone-sized screen. Submit a test entry using normal answers, then submit another using any fault or exception path. Confirm the entry is saved and the expected notifications arrive.
