Run this checklist before a form goes live
- Form title and instructions are clear.
- Required fields are genuinely required.
- Choice labels are consistent and understandable.
- Conditional fields show and hide correctly.
- Required fields hidden by logic do not block valid submissions.
- Uploads accept intended types and reject disallowed types.
- Signature capture works on a mobile device if used.
- Notifications go to the correct people and include the expected data.
- Confirmations show the correct next step.
- Entries can be found and reviewed by authorised staff.
- Page and file access are correct for each role.
- The form is usable on a small screen.
- At least one normal and one exception-path test entry have been checked.
Record the release
Record the form name/ID, test date, tester, changes made and any outstanding issues. Keep a known-good export of the form JSON before major changes.
