Build a Fault Report Workflow

Recommended workflow

  1. Create a form named “Equipment Fault Report” or use your approved existing form.
  2. Collect the operative, date/time, site/job, asset ID and a clear description of the fault.
  3. Add a severity or immediate-risk question only if your organisation has defined what each option means.
  4. Add evidence photo upload with suitable file restrictions.
  5. Use conditional logic to show follow-up questions only when needed.
  6. Configure a supervisor notification with the asset ID and fault summary.
  7. Configure a confirmation that tells the submitter the report was received and reminds them to follow the site safety procedure.
  8. Ensure the entry is visible to the people responsible for triage and resolution.

Test cases

  • Normal submission with no attachment, if allowed.
  • Submission with a photograph.
  • Submission with missing required data.
  • Submission that should trigger a supervisor alert.
  • Submission by a user with the normal operative role.

Important

A notification is not a closed-loop maintenance process by itself. If CHKM8’s installed workflow does not provide assignment, status tracking and closure, define how a supervisor records those steps using the available tools.
