Build the form
- Create a form named for the talk, or use a standard reusable toolbox-talk form if your organisation has one.
- Add the talk title, date, site/job reference and presenter.
- Add the briefing content in a page or description area that users can read on mobile.
- Add an acknowledgement checkbox with clear wording, such as “I confirm I have read and understood this briefing”.
- Collect the attendee name and any required employee/operative identifier.
- Add a Signature field if installed and required by the process.
- Save, embed on the appropriate page and restrict access as needed.
Make the record useful
Use a consistent naming convention and record the briefing date. If the content changes materially, consider versioning the talk so the record identifies which briefing was acknowledged. Test that the submission records the attendee, acknowledgement and signature correctly.
Do not overstate the sign-off
A form can record acknowledgement but cannot prove comprehension or replace a face-to-face briefing where one is required.
