Outcome
Create a repeatable inspection form that records the operative, asset, date, condition, faults and evidence.
Build the form
- Go to Forms → New Form and name it clearly, for example “Chainsaw – Pre-Use Check”.
- Add a Name Single Line Text field. Make it required if the operative must identify themselves.
- Add an Asset ID Single Line Text field. Make it required if every item of equipment has a unique identifier.
- Add a Date field for the inspection date. Configure a default only if your installed field/version supports the intended behaviour, and test it. A default date must not prevent users correcting an inaccurate date.
- Add a Radio Buttons field for each inspection question. Use clear choices such as “Yes – serviceable” and “No – fault found”; do not rely on colour alone to communicate the meaning.
- Add a Paragraph Text field labelled “Description of fault”.
- Add a File Upload field labelled “Photograph of fault”, with allowed file extensions and a reasonable maximum size.
- Add a Signature field only if the Signature Add-On is installed and the sign-off is required.
- Save the form and configure its confirmation and notifications.
Show fault fields only when needed
- Select the “Description of fault” field.
- Open its Conditional Logic settings and enable conditional logic.
- Choose Show this field if the relevant inspection answer is “No – fault found”.
- Repeat for the photo upload field, using the same condition.
- If several questions can report a fault, configure and test the logic carefully. Use “Any” when any one fault answer should reveal the section; use “All” only when every rule must be true.
Operational checks
Test one fully serviceable submission and one faulty submission. Confirm that the fault description and photo fields appear only on the fault route, that required validation works, and that the submission is visible to authorised staff. A form submission does not itself prove that equipment has been isolated or repaired; follow the organisation’s safety procedure.
