Prerequisites
The official Gravity Forms Signature Add-On must be installed and activated. It also requires compatible server support, including PHP GD support for signature image creation. The custom CHKM8 signature display component, if present, is separate from the official Signature field.
Add the field
- Open Forms and edit the form.
- In the Advanced Fields area, add Signature.
- Label it clearly, for example “Operative signature” or “Supervisor sign-off”.
- Configure appearance settings such as background, border and size if available.
- Set the field as required only when the workflow requires a signature before submission.
- Save the form.
Test the sign-off
- Preview the form on a desktop and a touchscreen mobile device.
- Draw a test signature and submit the form.
- Open the resulting entry and verify that the signature image is visible.
- Check any email notification or PDF/report workflow that is expected to include the signature.
- Confirm that authorised staff can access the record and that ordinary users cannot see other people’s submissions.
Important distinction
A captured signature is a record of the user’s action; it does not by itself verify identity, confirm that a person held a particular qualification, or establish that all legal requirements for a particular sign-off have been met. Define the sign-off wording and access controls for the specific process.
